To attend a Chrysalis Flight in the Nebraska Community, you will need to complete this Application and have a Sponsor.  Your Sponsor needs to be someone who has attended a Chrysalis or Emmaus weekend before, or another 3 Day qualifying event (Click here for a list of qualifying events).

It is recommended that your Sponsor is with you while you complete this online registration, either in person or on the phone.

There are several sections to this form.  Click on the next button at the bottom of each section to proceed to the next section.  You may have to scroll the form to see more questions and to reach the bottom where the next button is located.  There should be a scroll bar on the right side of the form inside the page.  Use that bar to scroll the form.  The scroll bar to the far right only scrolls the page.  They are two separate scroll bars.  Please be aware of this.  It is recommended that you be on a desktop or laptop computer and not use a pad or mobile device.
